Formats Available |

Rotary
International's membership card is available from Parker Systems in two formats (both
finished cards are credit card size):
- An Imprinted Plastic
Card - each card is imprinted by Parker Systems with
the member's name and other relevant information on the
face and a signature panel on the back - $1.99 each
(minimum order - one card),
- A Laser Printable Card - this format was designed to be
printed on a laser printer or typed on a typewriter
- $0.99 each (minimum order - 10 cards).

| You may order
your membership cards by filling out the On-Line
Order Form, then printing it for your records and
sending it to Parker
Systems, Inc., or if you prefer, faxing the printed form
to Parker Systems at (847) 726-8610. The Fax number and the Mailing Address for
Parker Systems, Inc. is on the Order Form for your
convenience.
